Login to blogger blog via this linkwww.blogger.com, Create a new blog, give proper blog name and URL. 

Google sometimes does not approve AdSense application for the newly created blogger account. It requires the blog to be atleast six months old.

OR

You can log in to your old blogger account.

Note: If your old blog is six months old than you can easily create a new blog and get adsense for that blog.

Now go to Earnings tab-> Signup for AdSense-> click Submit and AdSense widget will be added to your blogger blog.

Note: Google AdSense has recently changed their policy, now the blog should be atleast 6 months old and should have some good and unique content with some traffic from search engines.

Go to Layout and add AdSense widget to your blog layout. Select different ad units to show on your blog.

How to Display Google AdSense ads on a Custom Domain

Once your account is approved, Login to AdSense Dashboard.

Go to Account settings (left) -> Access and authorization->Sites authorized to show ads

Here you can see your account is enabled to show ads on YouTube and hosted sites (blogger) only.

Click on "edit" option and enter url of your website. The url should be a custom domain name. Submit this url to Google AdSense team. 

Now create an ad unit from My ads section. Copy the ad code and place it on your website.

Once they verify your website fulfilling all policies, your request will be accepted.

Note: If your website does not fulfill Google AdSense policies than your request may not be accepted.
